Objective. To evaluate the incidence and prevalence of SARS-CoV-2 infection in isolated municipalities of Córdoba, Colombia, and to develop more efficient real-time PCR diagnostic capabilities. Materials and Methods. A descriptive cross-sectional study was conducted in four municipalities of Córdoba, using samples from nasopharyngeal/oropharyngeal swabs and a real-time PCR detection kit. Statistical analysis was performed to describe population characteristics and determine associations. Results. Samples from 18,079 individuals were obtained, mainly from Sahagún. 2.2% tested positive for SARS-CoV-2. Approximately 50% of the sampled individuals were aged between 18 and 49 years, mostly rural women with no established ethnic affiliation. Conclusions. The study provides information on the incidence and prevalence of SARS-CoV-2 infection in isolated municipalities of Córdoba, Colombia. It highlights the importance of developing more efficient diagnostic capabilities. Findings will guide control and prevention strategies in these areas. Essential diagnostic and surveillance tools are needed to address the spread of the virus and protect public health in isolated communities.
